Post-Discharge Meals
After an inpatient hospital or skilled nursing facility stay, members can receive 2 meals per day delivered to their door for 14 days.
As a member of CHPW Medicare Advantage, a free gym membership is included in your plan coverage. The Silver & Fit program provides basic membership at a participating fitness center near you. Stay active and social by taking a fitness class. Regular exercise keeps you feeling better and supports your overall health.
